Each recommendation must include the individual’s name and contact information, position for which he or she is being recommended, and complete information regarding the individual’s Christian commitment, qualifications, and willingness to serve if nominated. Please adequately introduce your nominee to our Committee so that we might understand the person’s qualifications for service and the expected contributions he or she might make to the board or committee. Feel free to add an additional page in order to provide sufficient information when necessary.

What is the Annual Statistical Report?
Each year, ARP Central Services asks all ARP churches to provide information about their membership and finances. This information is collected so that the denomination can prayerfully plan for the future, as it guides denominational planning, growth efforts, and wise stewardship of resources. Thank you for your faithfulness in completing this important report.
When is the Annual Statistical Report Due?
On or before February 1, 2026
What Do I Need to Complete the Report?
Copy of your 2024 Annual Statistical Report for reference (if you need a copy, please contact Kristen Livingston at klivingston@arpsynod.org)
How Do Churches Complete This Report?
We offer three ways to complete and submit your Annual Statistical Report. Please choose one of the options below.
- Option 1: Complete Online Through ACCESS (ACS) – easiest/ preferred method

- If you would like to see an overall copy of the report to guide you in your submission, please use the 2025 Statistical Form posted below as a guide.
- Click this link to complete through ACCESS (ACS): https://secure.accessacs.com/access/eventlogin.aspx?id=VlC6jEZm1RWOw1aIO1i9tg==&site=96075&ReturnUrl=events%2fwz_people.aspx&ChurchID=2398&EventID=239139&sn=96075
- Under Sign Up, enter your email address and click Continue (in blue).
- Enter your First and Last Name and click Add.
- Click Next and follow the prompts to complete the form.
- Submit when finished.
- After submission, you will receive an email link with your completed report
- Use your email and password confirmation number to sign in
- You may print or save a copy for your records

- Open the attached EXCEL file titled 2025 Statistical Form
- Complete all fields highlighted in yellow
- Save as EXCEL file
- Email completed file to Kristen Livingston at klivingston@arpsynod.org
- Option 3: Call for Help Completing the Report

- If you have difficulties or do not have access to a computer, gather your statistical information and call Kristen Livingston and she will complete the report with you over the phone
- Call hours: Monday – Thursday, 10:00am – 3:00pm
- If these hours do not work, email klivingston@arpsynod.org to schedule a different time.

New Hires
New Hire & Change Collection Form
Click here to download the New Hire & Change Data Collection Form. This form is to be completed for all new employees, whether full-time or part-time, and returned to Care@forumbenefits.com. This form can also be used to report terminations, and changes in employee status, employee address, phone number and email.
